For your convenience, admission forms can be submitted in various ways. Receiving the completed admission forms prior to your arrival, allows us to better prepare for your pets accommodation and reduces the administration time on your arrival.

Admission forms can be downloaded, printed and completed prior to your arrival at Blue Mountain Kennels. These completed forms can then be either brought with you on arrival at Blue Mountain Kennels, or scanned and e-mailed to us at bmkennels@iafrica.com.

REQUIRED DOCUMENTATION

We also require the following documents, please bring them with, so that we can make copies for our records. Alternatively you can also scan and e-mail them to us prior to your arrival.

• 2. Copies of pet's vaccination certificates.
